VIDEO-ASSISTED LUNG SURGERIES

Traditionally lung surgeries were  done through a large cut either on the side of chest or front of chest. This entails much longer stay in the hospital, pain ,bleeding and possibility of infection. This is otherwise called thoracoscopic surgeries or VAT surgeries.

Vedeo-assisted lung surgeries are done through few small cuts 1-2 cm each, introducing a camera ,visualising in the monitor and accomplishing same procedure with minimum pain, least chance of infection,etc

 

                                     

 WHAT ALL SURGERIES CAN BE DONE THROUGH VAT ?
  • Stapled lung biopsy
  • Lobectomy or pneumonectomy
  • Resection of peripheral pulmonary nodule
  • Evaluation of mediastinal tumors or adenopathy
  • Pleural biopsy
  • Bullectomy
  • Treatment of recurrent pneumothorax
  • Management of loculated empyema
  • Pleurodesis of malignant effusions
  • Repair of a bronchopleural fistula
  • Chest trauma (mainly diaphragmatic injuries)
  • Pericardial window
  • Sympathectomy
  • Truncal vagotomy

WHAT ARE THE ABSOLUTE CONTRA-INDICATION FOR VAT ?

  • Markedly unstable or shocked patient
  • Extensive adhesions obliterating the pleural space
  • Prior talc pleurodesis
WHAT ARE THE RELATIVE CONTRA-INDICATION  FOR VAT ?
  • Inability to tolerate single-lung ventilation
  • Previous  chest surgeries
  • Extensive pleural diseases
  • Coagulopathy
  • Prior radiation treatment for thoracic malignancy; plan to resect
